Your 2025 1095-C Form is Available

This notice serves as confirmation that your IRS Form 1095-C, "Employer-Provided Health Insurance Offer and Coverage," for the 2025 tax year is available for your records. Due to recent changes in federal law, we are no longer required to automatically mail this form to you.

What is Form 1095-C?This form provides information about the health coverage offered to you by the Pleasant Valley Community School District during the 2025 calendar year. You do not need to file this form with your tax return, but you should keep it with your other tax documents.
 

How to Get Your Form:

ONLINE:

  • Log in online through the PVCSD Website at www.pleasval.org. On the very top of the webpage at the end of the Blue Bar you will see “STAFF RESOURCES” → hover over that and you will see “more” → click on “more” → it will take you to the staff resources page and under technology links you will see “weblink” → click on “weblink”
  • Enter your user ID: first four letters of your last name followed by the first 4 letters of your first name. Note: If you are an employee with access to Software Unlimited (our accounting program) your log in will be the same as your Software Unlimited username and password.
  • Enter your password: last 4 digits of your social security number or your Software Unlimited password.
  • Once logged in, go to Employee Information→Payroll and Compensation→1095’s. Once there you can check the box agreeing to the above consent terms and to receiving your 1095 through Weblink only.
